We are looking for two discharge coordinators to complement our existing ICT North and West teams. We seek individuals who can provide continuous coordinated support for the safe and timely discharge of patients from the caseload, working collaboratively within the Intermediate Care Team (ICT). The post holder will coordinate timely patient referrals to relevant community services and liaise directly with patients and their relatives about discharge arrangements. Additionally, the post holder will need to liaise with other key stakeholders integral to the discharge process. The post holder will support discharges over a 7-day period, requiring some weekend work on a rota basis.
About ICT - The ICT team aims to give rapid assessment, treatment, and support to patients who have recently declined in physical health and are at risk of hospital admission. This is achieved by supporting people in their own homes or helping them leave the hospital. We plan results that work towards maximizing independence, typically assisting patients in returning to their homes. We also deliver intravenous therapy to patients within their homes, ensuring they can stay out of the hospital.
Main duties of the job
1. Attend Team MDT meetings, with prior preparation to discuss complex cases and longest days on the caseload with the senior team.
2. Complete discharge notifications for onward referrals to other discharge services.
3. Complete all referrals to other services such as community nursing, adult social care, therapy, equipment, and voluntary services.
4. Communicate with patients who may have communication difficulties, families, and other colleagues.
5. Liaise with patients and give advice where appropriate regarding their care.
6. Ensure all patient documentation and pathways are documented electronically, following protocols for data collection purposes.
7. Input all discharge and admission data to support KPI and monitoring.
8. Be involved in the induction of new colleagues and provide support and guidance as necessary.
9. Proactively manage patients with no reason to remain on the caseload for more than fourteen days.
10. Proactively manage and allocate routine band 5 nursing visits to ensure flow through the caseload.
11. Review the package of care waiting list to identify delays and ensure patients are discharged in a timely manner to appropriate services.

Knowledge & Experience
* Level 4 NVQ or equivalent experience in a health-related subject.
* Experience working within a Healthcare setting.
* Functional skills in literacy and numeracy to level 2 or equivalent.
* Experience working in a community setting.
* Previous knowledge of SystmOne.
Skills & Abilities
* Excellent IT skills with the ability to use different electronic systems.
* Ability to work without direct supervision.
* Excellent time management and organizational skills.
* Ability to deal with competing priorities.
* Ability to work in a fast-paced and challenging environment.
