Key duties and responsibilities:
1. Carry out long term condition annual reviews delegated by a supervising Nurse or senior colleague.
2. Assist in and perform routine tasks related to patient care.
3. Take vital signs and observations, identify signs of improvement, and report any deteriorations or concerns, e.g., pulse, blood pressure, respirations, oxygen saturations, peak flow, blood glucose levels, assessing nutrition and hydration, and undertake pathology tests, such as urine culture and swabs.
4. Undertake wound care and management, as required.
5. Administer medications as prescribed and in accordance with NMC standards, e.g., via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions.
6. Follow agreed clinical protocols with referral to senior nurses or GPs as appropriate.
7. Assist in the assessment, planning, development, implementation, and evaluation of programmes to promote health and well-being and prevent adverse effects on health and well-being.
8. Deliver opportunistic health promotion using opportunities such as new-patient medicals and NHS Health Checks.
9. Document consultations accurately and appropriately, according to NMC guidelines.
10. Ensure awareness of statutory and local clinical adult and children safeguarding protection procedures, including systems of referral.
11. Recognise signs and symptoms of abuse/neglect.
12. Complete all mandatory training on an annual basis including safeguarding, confidentiality, equality and diversity, etc.
13. Recognise and manage anaphylaxis according to current UK guidelines.
14. Perform cardio-pulmonary resuscitation according to current UK guidelines.
15. Obtain and document informed consent (either written or verbal), as required.
16. Ensure infection prevention and control guidelines are maintained.
17. Chaperone and assist patients, where appropriate, who are being examined by another clinician.
18. Ensure confidentiality of information gained at work is preserved at all times.
19. Assist in practice audits, CQC compliance evidence and QOF recalls.
20. Participate and contribute towards any local incentive project relevant to the role.
21. Additional tasks following training and support will include: Cervical cytology, Adult and childhood immunisations & vaccinations, Learning Disabilities annual review.
22. Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).
23. Ensure accurate notes of all consultations and treatments are recorded in the patients’ notes on the clinical computer system as appropriate.
24. Attend and participate in meetings as required.
25. Ensure effective relationships and communications with colleagues and other agencies.
26. Identify potential changes to clinical practice that are required to implement evidence-based guidelines.
27. Contribute towards the education and training of phlebotomists, health care assistants, and those new to Primary care roles.
28. Advise patients on general health care with referral to GPs as necessary.
29. Monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment.
30. Participate in audit where appropriate.

The post-holder will acquire and maintain an acceptable level of knowledge and understanding of the Practices protocols, policies and procedures. Having acquired the required level of knowledge and understanding of the Practices protocols, policies and procedures; the post-holder is required to support, promote and adhere to all of the Practices policies.
Confidentiality
In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, practice staff, and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded and managed as strictly confidential. Additionally, the post-holder will be respectful and courteous to colleagues and patients, for example, knocking on doors prior to entry.

1. Using personal security systems within the workplace according to practice guidelines.
2. Identifying and reporting risks and potential risks involved in work activities and managing such risks in accordance with agreed procedures.
3. Maintaining a level of knowledge, understanding and skills to meet the Practices expectations.
4. Adhering to the Practices fire safety and infection control policies and procedures.
5. Maintaining work areas, so that work areas are tidy, safe and free from hazards and infection.

Other
The post-holder will be expected and required to undertake and complete any other duties as deemed relevant to position and the business needs of the Practice. This job description will be subject to periodic review and amendment in consultation with the post holder.
