What you will do: - Manage a complex caseload involving patients with neurological conditions and strokes, ensuring each receives tailored and effective treatment. - Conduct thorough functional assessments at patients' homes, crafting care plans that promote independence and final quality of life. - Collaborate effectively with a multidisciplinary team, including physiotherapists, speech and language therapists, nurses, and social workers, to provide cohesive care. - Deliver targeted rehabilitation interventions to enhance mobility, cognitive function, and daily living skills, using risk assessments and adaptive strategies where needed. - Regularly monitor and evaluate patient progress, ensuring results align with clinical outcome goals. - Uphold meticulous record-keeping that adheres to the standards expected of HCPC and trust policies. - Play an active role in service development through audits, service evaluations, and participation in team meetings. - Offer supervision and mentorship to junior team members, students, or assistants when required. Requirements: - Must hold professional registration with the HCPC as an Occupational Therapist. - Possess at least 2 years of post-registration experience, preferably with a background in stroke and/or neurology. - A full UK driving licence is essential, along with access to your own vehicle for travel across Havering and Barking & Dagenham due to the community-focused nature of this role. - Demonstrated excellent communication, time management, and interpersonal skills, with the ability to work independently and collaboratively.
