The successful candidate will be responsible for assessing patients' needs and tailoring exercise prescription for individuals with various heart conditions. They will be pivotal in helping with the health and wellbeing of our patients and maximising their cardiac fitness potential. The role will involve working as an autonomous practitioner as well as part of the cardiac rehab team. This will be in hospital and community settings, as well as telephone clinics and patients' homes.
The ideal candidate should have a strong background in exercise science, physiology and anatomy and be able to use this knowledge to create safe and effective exercise programmes for clients with cardiac conditions. They must have experience in cardiac rehab and be qualified to degree level in Exercise Physiology, or physiotherapy. The candidate should possess excellent communication skills and have a passion for helping others achieve fitness and wellness goals.
This post is non-rotational; the successful candidate will work exclusively within the Cardiac Rehab team.
Main duties of the job
1. Develop and implement safe and individualised exercise programmes for patients with cardiac conditions.
2. Set appropriate exercise-based goals with the patient.
3. Teach cardiovascular and strength training exercises.
4. Monitor and track patient progress and adjust programmes accordingly.
5. Provide education on safe exercise and the benefits of regular exercise for cardiac health.
6. Maintain the equipment and a clean and safe environment.
7. Liaise regularly with the clinical specialist team regarding treatment progression and ongoing care of patients.
8. Maintain patient records accurately and legibly in accordance with Trust guidelines.

Person Specification
Qualifications
* Degree/Diploma in Physiotherapy or equivalent.
* HCPC registration.
Experience
* Demonstrable post-registration experience.
* Majority of clinical experience in the NHS UK.
* Experience of working as part of a multidisciplinary team.
* Evidence of a contribution to clinical education/supervision of less experienced staff, students or assistants.
Knowledge
* Broad range of clinical assessment and treatment skills based on current best practice.
* Demonstrates an ability to comprehend and work within policies on health and safety, risk management and data protection.
* Working knowledge of current NHS issues.
* Evidence of self-directed learning and willingness to participate in CPD.
