Tees Esk and Wear Valleys NHS Foundation Trust
An exciting opportunity has arisen for 4 x PCN CYP Mental Health Practitioners to work into our local Primary Care Networks (PCN) GP practices to support the needs of our local population living with mental health difficulties (children and young people). This is a new and innovative service supporting young people in primary care settings and working in a collaborative and multi-agency way to meet the needs of patients with a range of mental health needs.
GP practices have begun working together alongside community, mental health, social care, pharmacy, hospital, and voluntary services in the local area to form PCNs. PCNs signal a significant change in the way practices interact not only with each other but the wider health and social care system. This will enable care to be delivered to a defined patient population, in a different way to meet the needs of that population. This will be focused on prevention and personalization of care, making best use of resources collectively.
There are 4 posts across the Teesside area (Middlesbrough, Redcar, Stockton & Hartlepool) and these will be discussed and allocated following the interview.
