The post consists of administrative support for the liver surveillance/ community liver health check projects. This will include working with the data from the four provider trusts, and there will be information governance responsibility to safeguard the data. It will also include working closely with the project manager to update project plans and work with the peer support workers. It is an exciting opportunity to be part of a project which will improve care for patients across Lancashire and South Cumbria.
There will be a requirement to work with the clinical teams from all four provider trusts, also setting up working relationships with primary care teams, drug and alcohol services and prison healthcare systems.
The post holder will need strong communication organisational and data management skills and experience of working with NHS systems. It would be very useful to be proficient in Microsoft packages.

Once a surveillance programme has been agreed across the Cancer Alliance, the post holder will be responsible for maintaining data collection, and monitoring compliance with surveillance protocols.

MAIN DUTIES:
The post holder will work as part of a multi-disciplinary liver surveillance team comprising of Nurses, clerical administrators and Doctors.

The post holder will be involved with setting up systems, working closely with the data collection coordinator and required to meet tight deadlines for data submission.
Monitoring and clerical duties
1. Develop and maintain effective working relationships throughout the ODN and Cancer Alliance across Lancashire and south cumbria
2. Collect, collate and report routine and data adhering to strict deadlines
3. Support and actively develop patient literature and surveys
4. Collect and collate patient satisfaction surveys for the ODN and produce associated reports
5. Take an active part in team meetings, including recording and distributing minutes, and updating action logs and project plans
6. Attend local and national events to develop own knowledge, sharing best practice and ensuring the ODN is fully informed
7. Coordinate patient caseload between peer support workers and local hepatology teams across the four trusts
8. General administrative duties to support the Liver team and Hepatitis C ODN as appropriate.
9. To exhibit professional behaviour and attitude at all times and demonstrate excellent customer care skills with respect for privacy, dignity, security and confidentiality.
10. To organise patient records and carry out clerical duties as and when required. This is to include use Winscribe, Cerner and other systems required.
11. To answer the telephone in a professional manner, to take messages and follow them up with the appropriate person.
12. To maintain clinical and administrative records in line with Trust Policies.
13. Meeting strict deadlines for data submission and supporting the data collection and coordinator with accurate data collection and submission
14. Attend liver screening events when required
Co-ordination and tracking responsibilities
1. To plan and organise clinical workload, managing this efficiently and effectively considering clinical priorities.
1. Highlight any regular system bottlenecks and contribute ideas to resolve these.
2. Work closely with the Liver surveillance team and Service Improvement Leads to continually improve the patient journey.
3. Ensure data entry of patient details to the liver surveillance monitoring database and submission criteria
4. Pro-actively monitor the surveillance database in order to prepare reports for each provider trust flagging patients due liver surveillance
5. Pro-actively monitor the outcome of attendance of liver surveillance appointments
6. Liaise with clinical outreach teams immediately if a patient fails to attend an appointment and take necessary steps to follow up.
7. Pro-actively work with liver surveillance peer support team in advance of appointments where it has been identified that there is a risk of non-attendance / need for support and facilitate regular PTL meetings.
8. Liaise with clinical team to ensure that liver surveillance scans and blood tests are requested in a timely manner
MDT Support
1.Be a first point of contact for the report of abnormal surveillance results and escalate immediately
2.Support exchange of information with HPB/ Hepatology MDT where appropriate
Project Support
1. Escalate any issues within the projects to the project manager / Directorate Manager quickly and effectively
2. Identify any risks and escalate
3. Provide administrative support in booking meetings with stakeholders, engagement events, producing and circulating minutes, circulating key documents and updating changes in project plans
4. Collaborate throughout the ODN/ Cancer alliance to maintain a wide, and flexible support function
5. The Post holder will be expected to show improvised thinking and propose changes where appropriate
6. Support the submission of quarterly CQUIN reports, annual reports and other quality improvement processes as required
7. Adhere to the quarterly submission criteria and escalate or resolve any bottlenecks.
8. Be responsible for the delivery of specific actions as identified within the ODN/ Cancer alliance Projects.

2. Resource Management
1. To support the liver surveillance team in the ordering of stock and equipment for the liver surveillance outreach programme, providing cross cover with other members of the team where necessary
