We are looking for an experienced, highly motivated midwife who is passionate about patient safety. Your role will be fundamental in continuing to ensure a focus on patient safety across the department, as well as support the consolidation and delivery of the new patient safety incident response framework (PSIRF), within a supportive environment. Key to this role is the provision of expert support, advice and guidance on patient safety elements. The risk and governance senior midwife will bring excellent interpersonal skills, work in collaboration and partnership with frontline staff, the Directorate leadership team, clinical leads and key stakeholders across both the organisation and regionally to support the planning and delivery of the Three Year Plan, facilitating and implementing change within the Maternity Services. An important aspect of the role will be to ensure that the Trust continues to meet with key performance indicators and compliance with national standards, supporting the agenda aligned to Better Births, Saving Babies Lives, MSDS, The Maternity Incentive Scheme and the CQC action plan.

Main duties of the job
* To work as a core member of the maternity patient safety team in supporting and delivering the national and local patient safety agendas and the patient safety incident response framework.
* Act as lead co-ordinator for designated incidents (clinical and non-clinical) ensuring that an appropriate review and/or internal investigation is carried out within the required timeframes.
* Assist the relevant leads to ensure that action plans derived from safety incidents/national reports are delivered.
* Ensure that themes from incidents are reviewed on a regular basis to ensure learning is shared and actions completed.
* Assist the department in ensuring that there is compliance with duty of candour and that there is commitment from all professional groups in order to further develop a positive culture of safety, which is open and honest.
* Ensure compliance with the governance assurance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement.

* In conjunction with the Director of Midwifery/Associate Director of Operations/Clinical Director and Quality and Clinical Effectiveness Midwife lead and advise on all aspects of Risk Management and patient safety.
* As an integral part of the Obstetric Governance Team, facilitate, develop and coordinate clinical risk management within the Directorate.
* Foster a fair blame culture in line with the corporate objectives, resulting in the delivery of a high-quality, cost-effective service.
* Promote a positive learning environment where lessons are learnt, and changes implemented to ensure a quality service working in partnership with the Director of Midwifery/Associate Director of Operations/Clinical Director/Quality and Clinical Effectiveness Midwife and Matrons.
* Deputise for the Quality and Clinical Effectiveness Midwife when required.
* To work regularly with colleagues in the clinical areas to:
o ensure professional midwifery skills are maintained
o to work closely with colleagues to raise the profile of Risk Management
o to highlight in practice the relationship between Risk Management and everyday clinical practice and the effects on women and families
o align complaints and litigation to incident investigation and share the emergent learning
o to identify areas of good practice and highlight areas that can be improved

* Registered Midwife
* Recognised teaching/assessing qualification or equivalent
* Extensive experience as a registered midwife
* Evidence of ongoing professional and personal development to Degree level
