Job summary This role will be for a Community Support Worker, working in our Dementia Intensive Support Team East. We are based at Walker Close Resource Centre in Ipswich and cover east Suffolk. Please note that this is a community based role, the successful candidate will need to have the ability to travel independently. You will be joining a team who have a passion for working with older people. DIST is to prevent avoidable/inappropriate admissions to acute hospitals and to support with discharges. The team supports people with dementia or suspected dementia who are at risk of admissions caused by Biological and Psychological Symptoms of Dementia such depression, anxiety or challenging behaviour. The diverse range of specialities in the team allows us to work collaboratively with the aim of providing high quality and integrated care. Our team works with our service users to help recovery and make a positive difference to their health and wellbeing. We believe that working closely with our service users and their family or carer allows us to provide bespoke care planning to meet their needs and wishes both in organic and functional health. Main duties of the job Older peoples nursing can be overlooked, however the complexity of the skills required for care for a patient with dementia embodies what mental health care is about and pulls on every facet of our training and knowledge base. You will be become an essential part of our multi-disciplinary team of Clinical Team Manager, Non-Medical Prescribers, Nurses, Nursing Associate, Assistant Practitioner, Care Support Workers and Mental Health Social Worker all supported by our fantastic admin.
