Minimum Qualifications of Education and Experience
A bachelor’s degree from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.
OR
A bachelor’s degree from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver’s license and be free of any major traffic violations for the last three (3) years.
Desirable Qualifications:
Two years of social service-related experience.
Nature of Work
Expected starting salary is $33.45 to $44.66. Full salary range is $33.45 – $55.87.
Under supervision of a Program Manager or Supervisor, and within Federal, State and locally developed programs and policies, supporting individuals diagnosed with a disability. Identifies individual needs and works toward meeting these needs through county and community support services. Will be working within a multi-disciplinary team setting. Supports a diverse, respectful, and inclusive workplace. The services help a person live as independently as possible in community settings and promote optimal health, independence, safety and community integration.
Examples of Work
1. Provides assessment and case management for individuals served through Waiver Programs.
2. Uses revised MN CHOICES assessment tool to determine the functional needs and abilities of the individual to determine eligibility for services.
3. Works with a variety of funding programs/sources (County, State, and/or Federal) to meet the individual’s needs.
4. Develops and writes community support plans.
5. Advises individuals, and their families of possible resources, rights and responsibilities under programs or services.
6. Works with individuals, team members, and providers who support the individual.
7. Refers individuals to other programs or agencies when appropriate.
8. Utilizes State and Local computer programs to document work for the county and the individual.
9. Maintains case records in accordance with State, Federal, and local guidelines.
10. Performs all other duties as assigned.
Knowledge, Skills and Abilities Required
1. Knowledge of a variety of program and services for individuals with disabilities or related conditions, including knowledge of physical health, mental health, and chemical health needs.
