Job Summary:
The Providence Center Case Manager - Pawtucket Health Home will coordinate services for consumers to minimize stressors and facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence.
Duties and Responsibilities:
1. Coordinate a variety of services.
2. Treat consumers with respect.
3. Provide support counseling, problem solving, contracts, and limit setting.
4. Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, including working with substance abusers, as it relates to case management.
5. Make assessments and provide appropriate crisis interventions and risk management as needed.
6. Teach consumers adult daily living skills.
7. Integrate skills training in all aspects of Rehabilitation and Recovery Plan.
8. Initiate treatment and advocate for consumer within network of care.
9. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
10. Transport consumers in own vehicle or Center van to planned activities and appointments.
11. Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
12. Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
13. Attend trainings, case presentations, and conferences.
Requirements:
1. Bachelor's degree from an accredited four-year College or University in Human Services or related field.
2. Experience in Human Services field preferred.
3. Must have a valid driver's license and insured auto.
4. Must be able to be flexible.
