Job summary As a new ARRS funded role, the PCN are looking for a dynamic, enthusiastic and forward-thinking individual, who works well both independently and as part of the wider Multi-Disciplinary Team (MDT). The GP will be provided with a named supervisor throughout the duration of the post. The funding for this role is currently guaranteed until March 2025, however there may be possibilities to continue working with the Network beyond this date. In line with the funding requirements, the post-holder must be a newly-qualified GP (that is within two years of qualification and having not held a substantive post). Main duties of the job The purpose of this role is to support the PCN in delivering high quality care to the patients it serves. The role is intended to support the PCN and its practices in the delivery of their contract, which will require the post-holder to complete: Face to face and telephone consultations Home visits and Care Home visits Checking and signing repeat prescriptions Administration - dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely manner Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE About us NN2 is a Primary Care Network (PCN) in North Norfolk, which is made up of five GP Practices: Aldborough Surgery, Birchwood Medical Practice, Cromer Group Practice, Mundesley Medical Centre and Paston Surgery. The PCN Team and Practice Teams consist of a number of roles, including Advanced Practitioners, Clinical Pharmacists, Pharmacy Technicians, Care Coordinators, Physician Associates, GP Assistants, Mental Health Nurses, Health and Wellbeing Coaches, and Social Prescribers. The PCN work with a range of local providers, including: community services, social care, and the voluntary sector, to offer more personalised, co-ordinated health and social care to the local population. Supporting patients to adopt health strategies that promote healthy lifestyles and apply principles of self-care, including providing counselling and supporting guidance on healthier living.
