First Contact Practitioner
Buckinghamshire Mind is looking for a First Contact Practitioner. The First Contact Practitioner is responsible for providing an enhanced triage assessment for patients seeking mental health support within a Primary Care Network (PCN) and plays a key role in ensuring timely access to appropriate mental health services while reducing pressures on GPs. The purpose of this role is to offer early intervention, improve patient pathways, and provide high-quality mental health support in a primary care setting.
* Contract: Type: Fixed Term until 6th October 2025 but potential for this to be extended.
* Location: Mid-Chiltern PCN (Rectory Meadow Surgery, Amersham Health Centre, Hughenden Valley Surgery, Prospect House Surgery and John Hampden Surgery). As this role is based in the community with travel between several locations, you’ll need access to a vehicle for use during work hours. Alternative transport arrangements will be considered. Travel expenses will only be paid for journeys that fall outside of the regular commute to and from the work location(s)
* Salary: £24,334 pa

Key Responsibilities
* Provide mental health triage assessments for patients presenting at GP Surgeries.
* Support individuals with a range of mental health concerns, offering facilitated access to relevant services.
* Work collaboratively with GPs, Advanced Nurse Practitioners, and other healthcare professionals to deliver high-quality mental health care.
* Be responsible for a caseload of patients, assisting them to use the guided self-help model.
The ideal candidate will have
* Experience of working in mental health services or a related healthcare setting.
* Strong assessment and triage skills to determine the best care pathways for patients.
* Ability to work autonomously within a multi-disciplinary team.
* Excellent communication and organisational skills, with a patient-centred approach.
