Community Mental Health Practitioner (Nurse/OT/Social Worker) - ILS
Band 6
Job overview
We are offering an exciting opportunity for a Band 6 Community Mental Health Practitioner to work within our Intensive Life Skills Pathway based in South Staffordshire (Lichfield and Tamworth, Burton and Uttoxeter, Cannock, and Stafford and Seisdon). This service is expanding, and it is an exciting time to join the service as a highly skilled NMC Registered Nurse, HCPC Registered Occupational Therapist, or Registered Social Worker.
The Intensive Life Skills Pathway is a multi-disciplinary Mental Health Team providing assessment and intervention for adults experiencing complex emotional difficulties as part of the Adult Mental Health Service. The team works in a highly psychologically minded way and are enthusiastic about working with service-users who present with high levels of emotional distress and often have histories of complex trauma and/or adverse experiences.
The pathway uses Structured Clinical Management (SCM) as a core therapeutic model, combining individual and workshop-based interventions. A range of additional therapeutic models are utilised including attachment theory, behavioural theory, Compassion-Focussed Therapy, EMDR, Trauma-Focussed Cognitive Behavioural Therapy, Mentalisation-Based Treatment, and DBT informed interventions. An understanding of this model would be desirable, but training, support, and supervision will be offered within the service. You will need to travel independently over a large geographical area and will require suitable transport.
Main duties of the job
As a Mental Health Community Practitioner you will be expected to undertake holistic comprehensive assessments and formulation of complex clinical presentations, including risk assessments and risk management. The role will involve:
1. Delivering group-based and individual interventions in line with the pathway model and utilising Structured Clinical Management (SCM). There will be a requirement to undertake the role of named worker for a caseload of service users.
2. Some individual and group-based interventions are being delivered remotely, and the successful candidate will therefore have strong ICT skills and be able to communicate effectively with others through technology.
3. Participate in both delivering and receiving a range of clinical supervision and professional and clinical development within the pathway.
4. Responsible for care coordination of an identified caseload.
5. Provide consultation, support, and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate.
