Job summary The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care. The role requires clear communication between the Hospital Discharge Team, the Discharge to Assess Hub and the wards to ensure safe timely discharge of all patients.
