Jessie May delivers community-based palliative nursing care to families caring for children and young people with life-limiting and life-threatening conditions across various locations including South Gloucestershire, Bristol, North Somerset, BANES, Swindon, and Wiltshire. The service delivery hours are 08:00-20:00, including weekend, bank holiday, and overnight on-call as part of the 24/7 end-of-life care provision.
The post holder will take delegated responsibility for a specific geographical area, leading and coordinating the day-to-day activities of the Care Team within this area. They will deliver skilled, holistic nursing care and guidance to children and young people with complex medical needs while supporting their families. Responsibilities include planning, implementing, and evaluating individualized care to ensure the highest standards of care are met.
Main duties of the job
The post holder will possess excellent communication skills and demonstrate the ability to build and maintain strong relationships with the children and families on the caseload. They will work collaboratively with a wide range of stakeholders, including healthcare staff, primary care teams, social services, and other relevant professionals. Their role will be pivotal in ensuring a coordinated, multi-disciplinary approach to care.

Knowledge and Experience
* Experience of taking the lead in supporting staff within their role
* Knowledge of family-centered care and the challenges faced by families on a palliative care pathway
* Relevant post-registration experience, including palliative care and/or end-of-life care
* Evidence of team working and effective communication with the multidisciplinary team
* Knowledge of safeguarding and appropriate actions to minimize risk
* Experience of needs assessment and care planning for complex needs
* Experience of managing a caseload
* Experience of lone working in the community with children, young people, and families
* Experience of developing and promoting a service to a new geographical area
Skills and Abilities
* Ability to organize and prioritize own/team’s workload
* Excellent clinical skills
* Excellent communication & interpersonal skills
* Able to manage and make clinical decisions for allocated and shared caseload
* Car Driver, mobile across the geographical area served
Qualifications and Training
* RSCN/RN (Child)
* Educated to Diploma or Degree level or working towards this
* Community Children’s Nursing qualification
