Job summary We are looking for a dynamic and motivated Senior Clinical Pharmacist to manage our clinical pharmacy service provided to the acute medical admission wards and the Emergency Department (ED) at York Hospital. Reporting to the Principal Pharmacist for Medicine (Acute & Frailty) medicine you will be supported to implement systems to improve the safe prescribing and transfer of care of acutely admitted medical patients. Whilst based in York, we encourage cross-site collaboration with counterparts at our Scarborough site to ensure a consistent service across the Trust. Supported by a team of specialist and rotational pharmacists plus medicines management technicians and assistants, you will work at a senior level and play an influential role in the medical patient admission care pathway. You will use proven clinical skills help deliver a high-quality patient focussed clinical pharmacy service with the scope to innovate, audit and make quality improvements to our processes. We welcome interest from pharmacists from all backgrounds but you must hold a postgraduate qualification in clinical pharmacy to Diploma level and be an Independent Prescriber (or working towards this). Experience of effective engagement with patients, the wider MDT and pharmacy team members is vital for this post. The combination of responsibilities within this role presents an ideal opportunity to develop both your clinical and leadership skills. Main duties of the job Through demonstrating competence in technology and improvement, you will review and monitor medication usage for the ED and the acute admission units, including the management of the Omnicell stock cabinets in ED. Ensuring the safe & secure handling of medicines, you will advise on compliance to formulary guidance, improve prescribing processes and address day to day medication queries, stock issues and efficiencies. Excellent communication and interpersonal skills are essential as a strong commitment to education and training of junior pharmacists will be required to support and develop the service. The post includes participation in working the weekend (1 in 8), bank holiday (1 per year) and on-call rotas. On-call working frequency is approximately 1 in 30 shifts.
