Service Location: YMCA of the North, St. Paul Midway and Minneapolis Dayton Y locations, 1761 University Avenue West, St. Paul, MN 55104 & 651 Nicollet Mall, downtown Minneapolis, 55402
Living Allowance: $8,074.00

PROGRAM PURPOSE:
The National Association of Chronic Disease Directors (NACDD) is a national public health non-profit agency focused on improving the health of the public by strengthening state and national leadership and expertise for chronic disease prevention and control. NACDD promotes health and reduces the burden of chronic disease through various programs and initiatives in collaboration with the Centers for Disease Control and Prevention (CDC).
SPECIFIC SERVICE FUNCTIONS AND RESPONSIBILITIES:
* Support ForeverWell Programs and Wellness Programs by promoting events to community members.
* Implement ForeverWell member engagement strategy by contacting inactive members.
* Create and lead ForeverWell events such as lunch and learn programs or healthy living seminars.
* Increase member awareness of Midway and Dayton Community Wellbeing Programs through flyers, newsletters, and table displays.
* Lead new member orientations and share complimentary services.
* Conduct eGym orientations and promote scheduling with Personal Trainer.
* Create a Community Health Fair by inviting organizations to set up informational tables.
* Design a Fall 2025 Wellbeing event.
* Develop community partners for ForeverWell work.

REQUIRED COMMITMENT:
Service members are expected to complete 1200 service hours within one (1) year, from 02/03/2025 to 02/02/2026.
DESCRIPTION OF ANTICIPATED SERVICE SCHEDULE:
In-person service hours at Midway YMCA and Dayton YMCA, four days a week at 6 hours per day, with flexibility and possible Saturday work.
ORIENTATION AND TRAINING:
Members must participate in pre-service orientation, PHA-specific training, program-specific training, and any other necessary trainings.
EVALUATION AND REPORTING:
PHA Service Members will complete quarterly assessments, monthly reflection forms, participate in performance evaluations, and submit service timesheets biweekly.
PUBLIC HEALTH AMERICORPS ELIGIBILITY AND REQUIREMENTS:
* Be at least 18 years of age at the commencement of service.
* Have a high school diploma or its equivalent.
* Be a citizen, national, or lawful permanent resident alien of the United States.
* Satisfy the National Service Criminal History Check eligibility criteria.
* Live within commutable distance of the Host Site.
OTHER PROGRAM ELIGIBILITY AND REQUIREMENTS:
Must comply with all Host Site vaccination requirements, including for COVID-19.
