Working alongside the clinical lead to screen, approve, review and case manage clients through to discharge - those requiring a dedicated period of specialist rehabilitation following an acquired brain injury.Working alongside specialist providers, clients and families to ensure quality outcomes, managing expectations and preventing delays in transfer of care along the pathway. Working knowledge of complex neurological conditions disease process and injury with respect to cognitive and physical impact required. Experience of working with clients with complex brain injuries that may have new or pre-existing dual mental health conditions requiring care under the Mental HealthActAcquired Brain injury case management experienceSpecific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction, e.g. NICE LTCSupport to the mental health assessment pathway may be required in this post.
