Role Description
This is a part-time Care Coordinator role located in Chelmsford. The Care Coordinator will be responsible for care coordination, rota management, communication with clients and their families, completing care for our clients, on call support and providing excellent customer service on-site.

* A full licence and your own car
* Effective Communication and Customer Service skills
* Experience in Home Care coordination
* Strong interpersonal skills and empathy
* Attention to detail and organizational skills
* Ability to work independently and as part of a team
* Knowledge of local community resources is a plus
