About the Job:
1. Assist registered nurses/health care professionals in the delivery of individualised holistic patient care according to policy/procedure.
2. Ensure that consent is obtained prior to delivering all aspects of patient care.
3. Involve relatives/carers in the delivery of care, interacting helpfully and in a friendly manner.
4. Deliver care with compassion and respect to all patients.
5. Work with the multi-disciplinary team to prevent patient harm by participating in risk assessments (e.g., pressure ulcer development, falls, nutrition, comfort rounds).
6. Maintain privacy and dignity at all times and protect confidential information.
7. Assist patients with all care as delegated by the registered nurse/health care professional.
8. Assist patients in meeting their nutritional needs and accurately record intake on the appropriate charts, reporting any changes to the registered nurse/practitioner or doctor responsible for the patient's care.
9. Safely and accurately measure and record patients' physiological observations (e.g., pulse, respirations, blood pressure, temperature) and escalate/report to the registered nurse/health care professional responsible for the patient's care.
10. Safely and accurately measure and record blood glucose levels, reporting to the registered nurse/health care professional responsible for the patient's care.
11. Accurately document any observations and care provided in the appropriate record, informing the registered nurse of your actions.
12. Safely undertake urinalysis, record results, and report to the registered nurse or doctor accountable for the patient's care.
13. Safely obtain samples (urine, stool, sputum, etc.) in accordance with Infection Prevention policies and guidelines.
14. Safely transport samples to laboratories in a timely and professional manner.
15. Safely escort patients to other areas/departments as requested by the registered nurse/health care professional.
16. Chaperone patients requiring clinical examinations as per the Trust's Chaperone Policy.
17. Use appropriate moving and handling techniques and equipment when manual handling.
18. Report back any relevant information to the registered nurse accountable for the patient's care.
19. Ensure the patient has a positive experience whilst in your care.
20. Answer the telephone courteously and professionally, accurately taking messages and informing the relevant person(s).
21. Respond appropriately to patient concerns and questions, escalating to the accountable registered nurse/health care professional as necessary.
22. Maintain cleanliness and tidiness in areas, ensuring appropriate stock levels, including storage areas and medical equipment.
23. Comply with Health and Safety Regulations and report accidents/untoward incidents/hazards/unsafe equipment to the nurse in charge/registered practitioner immediately.
24. Act in a courteous and helpful manner at all times, ensuring compassionate care while respecting others, including the care of deceased patients and their families.
25. Maintain professional behavior at all times as an employee of the Trust.
26. Work towards healthcare support workers registration with the Nursing Midwifery Council as established.
27. Attend mandatory training and courses as indicated by the needs of the service and personal development plan.
28. Familiarize yourself with and comply with all appropriate Trust policies and procedures.
29. Maintain a professional image and comply with the Trust's Dress Code Policy.
30. Uphold the reputation of the profession and the organization.
31. Undertake any other duties as may be required to meet the needs of the service within the limitations of the grade for the post.
