Lead Worker – Floating Support Services
Telford, Shropshire
£12.15 per hour FTE £23692.50 per annum
Various up to 37.5 hours per week
Must have full clean driving licence and access to vehicle
About the role
As a Lead Worker, you will provide early, targeted, and flexible support to individuals and families across Telford and Wrekin. You will work with young people aged 16-54 and older individuals aged 55+, supporting them to maintain their tenancies, achieve financial independence, and improve their health and well-being. This role focuses on homelessness prevention, tenancy sustainment, and promoting independent living.
Key Responsibilities:
* Conduct comprehensive assessments of customers’ housing, financial, health, and social support needs.
* Develop personalised, outcome-focused support plans tailored to individual strengths and goals.
* Collaborate with multi-agency partners to deliver coordinated and holistic support services.
* Provide practical assistance with budgeting, debt management, and accessing welfare benefits.
* Support customers in connecting with community services, reducing isolation, and fostering social inclusion.
* Uphold safeguarding standards, ensuring the health and safety of vulnerable individuals.
* Maintain accurate records, ensuring compliance with Key Performance Indicators (KPIs).
