Job Context
Weldmar Hospicecare is an independent charity that provides specialist end of life care to patients and those that are important to them throughout Dorset. This care is given in the comfort of patients' own homes by Weldmar Community Nurses and our Weldmar at Home Team, and via Wellbeing Support Sessions held across the county or as inpatients at our hospice in Dorchester. Our team of dedicated staff and volunteers ensure that we can continue to support patients and families through one of life's most difficult journeys. We work closely with other health care providers including the NHS to ensure that our patients and families receive the highest possible level of care.
We do receive some funding from the NHS but the majority of our income is generated by our fundraising and retail teams, which plays a vital part in securing income to fund our care, both now and in the future.
Job Purpose
Reporting to the Patient & Family Support Lead, the Wellbeing Support Coordinator will work as part of a team to deliver Wellbeing Support Sessions to patients, carers, or bereaved relatives, at the IPU or one of our community settings. To be responsible for the delivery of a delegated programme of Wellbeing Support Sessions, building rapport with and accounting for the varying needs of those accessing our services, to ensure the highest standard of Wellbeing Support. The Wellbeing Support Coordinator will be responsible for the day-to-day coordination of the Wellbeing Support Sessions, as well as contributing to the development of the Wellbeing Support Service, Wellbeing Support Worker, and Volunteers to ensure it is successfully working towards the achievement of Weldmar Hospicecare's aim, objectives, and values.
The post holder will work flexibly to ensure delivery of the service at our Inpatient Unit and at other community settings, responding to demands of the service.
Scope and Limits of Authority
Reporting to the Patient & Family Support Lead, the Wellbeing Support Coordinator will be responsible for:
* Leading the day-to-day operations and delivery of the service, including managing the facilities.
* Processing of referrals to Wellbeing Support Sessions, considering operational capacity and logistics, as well as individual needs and ongoing eligibility of those accessing the activities programme.
* Monitoring service users' wellbeing, including clinical care needs and overall safety, liaising with members of the multi-disciplinary team as necessary and escalating to the Patient & Family Support Lead as appropriate.
* Line management and development of the Wellbeing Support Workers, Creative Practitioner, Minibus Driver, and overseeing and deployment of Wellbeing Support Volunteers.
* Ensuring the accurate and timely completion of SystmOne records.
* To ensure understanding and adherence to Weldmar Hospicecare policies and procedures within the team.
Job Tasks
Service Delivery
Under the guidance of the Patient & Family Support Lead, the Wellbeing Support Coordinator will:
* Plan and deliver a range of activities on the Wellbeing Support Sessions programme.
* Contact all new referrals to the service, arrange initial visits and welcome new attendees to the sessions.
* Monitor attendance levels, being aware of capacities for each session, and advising new referrals to the service if there is a waiting list in place.
* Facilitate social activities for attendees and recognise the wishes and sensitivities of individuals in relation to all activities.
* Maintain a relaxed and safe environment for attendees, encourage and facilitate interaction between them.
* Act as a point of contact for each Wellbeing Support Sessions community setting, acting on any issues that may arise from the hire of each setting and escalating to the Patient & Family Support Lead as necessary.
* Uphold a culture of person-centered respectful care and work in a person-centered way focusing on the needs of the individual in line with Weldmar's aims, objectives, and values.
* Coordinate activities at Wellbeing Support Session setting, dealing with issues as they arise and escalating as required.
* Maintain a safe environment at all times, ensuring equipment is clean and safe, and reporting any hazards or concerns, to include, where appropriate, using the RADAR system.
* Be aware of individual risks and the requirements of associated risk assessments and ensure that these are adhered to and initiate appropriate action in an emergency situation.
* Coordinate transport for patients attending Wellbeing Support Sessions, who meet the Transport Eligibility Criteria as outlined in the Transport Policy.
* Act as Minibus Escort on the Minibus identifying any changes or significant mobility problems and taking appropriate action.
* Contribute to regular reviews of WSS attendees to ensure they are still eligible for Weldmar support, and are continuing to benefit from the service. Where discharge from the service is identified as the most appropriate action, to work with the service users to find alternative opportunities in the community, and signpost as appropriate.
Person Centred Care
* Provide a high level of emotional support for patients, carers, or bereaved relatives who may be in distress; using listening and empathic skills, offering the opportunity to speak privately with a member of the team if required.
* Provide hands-on care when needed to those patients attending requiring personal care assistance e.g. help with toileting and assisting with urinary catheters, including the competent use of moving and handling aids such as hoists.
* Liaise with Weldmar Community Nurses when identifying symptom management issues that need addressing.
* For those patients with complex physical needs, work in equal partnership with patients, their carers involved and members of the multi-disciplinary team involved in their care. Implement individualised care plans focused on empowering people, and to monitor and review outcomes.
* Ensure that accurate and up-to-date records on SystmOne, recording key information and ensuring records are written in a timely manner and adhere to Weldmar Clinical Recording Standards.
* Refer on to other professionals when required, for further support.
Leadership and Management
* Line manage the Wellbeing Support Team, to include regular 1:1s, supervision, appraisal and development.
* Support volunteer helpers ensuring they understand tasks and feel confident and valued in their role.
* Proactively participate in opportunities for shared learning and personal and professional development, including clinical supervision.
* Prepare staffing rotas in order to ensure the safe and effective delivery of the full range of sessions.
* Provide support and guidance in all aspects of Wellbeing Support.
* Ensure that adequate training is provided to ensure that competent use of SystmOne and other software packages.
Communication and Relationships
* Work in conjunction and foster relationships with colleagues and other departments (including administration team, WCNs, AHPs, IPU staff, catering staff, Hotel Services, community settings) to ensure smooth and safe delivery of services to Weldmar standards.
* Lead Team Meetings, encouraging team discussions and collaboration.
* Actively participate in 1:1s, identifying areas for own development.
* Participate in any opportunities for shared learning and reflection and identify any appropriate further training which would be beneficial to the role.
* Share any acquired knowledge from training courses, seminars or other sources with colleagues where appropriate.
* Participation in Weldmar's Clinical Supervision provision is encouraged.
