Job summary We are looking for a passionate and motivated Band 6 Occupational Therapist to join our Memory and Later Life Team. You will have the opportunity to work alongside experienced members of the multidisciplinary team within the three distinct pathways (Early Memory, Complex Care and Care Home Education and Support Service). This post offers a variety of opportunities within the service, with a focus working within care homes to provide specialist Occupational Therapy assessments and interventions. This role provides a great opportunity to utilise core skills of Occupational Therapy in providing specialist assessment and interventions, whilst working alongside the patients and their carers to promote engagement in meaningful occupation. You will be part of a hardworking and caring team providing specialist input to people living in care homes and the community, with cognitive difficulties, mental health needs alongside other physical health, neurological and complex conditions. Due to the nature of the role, you will be required to travel across the county to visit patients within care homes or their own homes. Main duties of the job To be responsible for the assessment and management of a caseload of service users who present with a variety of complex Occupational Therapy needs, diverse presentations and a range of mental health physical health, neurological conditions, cognitive and/or learning disabilities. To work primarily within the Care Home Education and Support Service (CHESS), but across all three pathways dependent on clinical need. In partnership with the Service Users and Carers where appropriate, develop collaborative intervention plans, ensuring regular reviews to monitor progress and adapt as necessary. To deliver evidenced based Occupational therapy interventions. Co-ordinate treatment planning & interventions, reviews and discharge planning. To work as part of the Multidisciplinary team alongside doctors, nurses, support workers and care home staff to provide high quality, co-ordinated treatment plans. Provide clinical supervision and leadership to staff as delegated by Clinical Lead. To provide compassionate care that is based on empathy, kindness, respect and dignity.
