An exciting opportunity has arisen for the right candidate to join our Ageing Well team.
As an Advanced Clinical Practitioner, you will take a lead role in assessing and working with adults who have complex and/or multiple long-term conditions, including frailty, with the aim to reduce hospital admissions. As an autonomous practitioner, you will use a case management approach to ensure the individual’s condition remains as stable as possible, reducing unnecessary hospital admissions and improving well-being.
You will be responsible for ensuring continued delivery of services, upholding the vision and values of the organisation, and ensuring the quality and safety of the services in line with CQC standards.
The post holder must have effective communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service. If you are innovative, motivated, dynamic, and forward-thinking, this might be the right post for you.
The post holder will be expected to be able to travel independently across the borough.
