Job summary We have exciting opportunities for the right candidate to join us within The Virtual Ward service as a Senior Health Care Support Worker. The members of staff will be required to function as a member of the multi-disciplinary team, assess individual patient's care needs and escalate concerns to the correct member of the team. The members of staff would be required to attend training to maintain mandatory skills alongside developing new skills to meet the provision of care required. Main duties of the job The post requires excellent communication skills, the ability to work flexibly and team working to provide quality care to patients with complex health care needs within their homes. The post holder must have QCF Level 3 or equivalent in care. The post holders will be required to work in the community and will require a driving license. You will need to provide a range of skills including delivering care to patients with complex health needs within their homes, supporting patients and their families. Desirable skills include the ability to perform venepuncture, simple wound care, basic observations and ECGs.The post requires a close working relationship with the Community Nursing Service to ensure that individuals needs are met as quickly as possible to enable them to remain at home with the appropriate level ofcare and support required.
