The Quality Improvement Lead reports to the Director of Care and takes the lead of the internal Quality Improvement Program and is very familiar with all Residents First Initiative. Co-chairs the Quality Team Meetings and coordinates the Quality and Risk Management Program and Resident Quality Inspection process (RQI) by fostering a culture of continuous quality improvement with a focus and emphasis on safety.
Responsibilities:
* Coordinate the Continuous Quality Improvement Program and the Resident Quality Inspection (RQI) process. Quality Team Leader.
* In collaboration with the Leadership Team, develop and submit the Annual HQO Quality Improvement Plan and Progress report for the Home.
* Lead the internal Quality Team Meetings.
* Train new staff on the RQI process and the Inspection Guides interview questions and surveys.
* Utilize the Health Quality Ontario website for resources to advance quality improvement activities.
* Ensure that all quality management and risk management related activities are reviewed, discussed, and follow-up actions identified as necessary.
* Monitor indicators on the public website of HQO and Your Health System.
* Communicate data analysis, trends, and Quality Improvement activities to Residents' and Family Council.
* Work together with the Accreditation Coordinator to ensure all quality and safety standards for CARF accreditation are being addressed.
* Understand the nature and meaning of quality indicators used by the Home and understand which indicators are publicly reported.
* Participate in the improvement of the indicators and achieve satisfactory results.
* Other duties as assigned and or as indicated in Job Task Inventory.
Qualifications:
* Comprehensive knowledge of health care practices, trends, and issues in long term care and impacts on other health care sectors/community at large.
* Excellent written and oral communication skills.
* Excellent knowledge of Quality/Risk Management philosophies and programs.
* Master trained in Resident Quality Inspection process.
* Training in Quality Improvement and Innovation preferred.
* Good understanding about the RAI-MDS 2.0 process and CIHI’s indicators.
* Working knowledge of relevant computer applications and applicable legislation.
* Positive working attitude.
* Strong interpersonal and team building skills.
* Ability to focus on people’s feelings when interacting and know how to turn a task into positive social interaction.
* Undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
* Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
* As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements.
* Two supervisory references required.
