Cleveland House is currently recruiting for a Care Home Assistant Practitioner to join their dedicated team in Southport. As a Care Home Assistant Practitioner, you will supervise Support Workers who assist our service users in achieving their outcomes in the way that they choose.
Job Responsibilities:
* Deliver prescribed care to a defined group of patients/service users that reflect their individual needs.
* Assess, plan and deliver, with the patient and their carer/family, care which takes account of patient choice and wishes.
* Contribute to a team approach to patient care in conjunction with all members of the Multi-Disciplinary team and liaise effectively with external professionals.
* Maintain accurate and contemporaneous records of patient/service user care delivered.
* Respect and apply the requirements of equality and diversity, promoting and role modelling these across the team.
* Develop and maintain a good professional relationship with patients, colleagues and visitors responding promptly and courteously to requests, enquiries, concerns, complaints and suggestions.
