To identify, review and optimise the treatment of patients with diabetes, developing management plans, initiating and titrating prescribed treatments and evaluating outcomes. To support other primary and community care practitioners towards providing excellent care To be a skilled practitioner providing expert clinical care using an extensive theoretical knowledge base and practical experience to underpin the role, whilst acting as a role model for the delivery of expert care. To deliver specialist educational programmes to a range of healthcare professionals in relation to Diabetes including medical, nursing and Allied Health Professional staff. The post holder will have robust links with partners in primary and secondary care, voluntary organisations and patients in order to provide seamless care for patients and their families. The post holder will work clinically alongside all members of the Diabetes Nursing Team, Consultants, GPs and wider multi-disciplinary teams to ensure the provision of effective, efficient, appropriate quality evidence based care to patients with diabetes.
