A part-time opportunity has arisen to join our specialist palliative care team.
The 20.5 hour post to work over 3 days is a Band 7 CNS role based in Y Bwthyn Princess Wales Hospital Specialist Palliative Care unit. The position provides an outreach service to patients with palliative care needs across the community and hospital setting, supporting the population of Bridgend and the surrounding valleys in the local authority area.
The specialist palliative care service is delivered within teams on multiple sites across Cwm Taf Morgannwg. The post holder may need to provide support to cover the CNS role across other sites, depending on patient needs.
The position involves directly supporting and advising patients with palliative care needs in both hospital and community settings. Multidisciplinary team working is essential to lead education and provide support and guidance to colleagues about the specialty. The core hours are Monday to Friday, 9-5 pm, with participation in a 7-day roster requiring the post holder to give advice to other healthcare professionals, patients, and families, potentially visiting patients in the hospital and community settings on weekends.
The successful applicant will be responsible for managing a caseload of patients with specialist palliative care needs in collaboration with the wider MDT, participating in education, audit, and providing leadership regarding the specialty.
You will need to be enthusiastic about the specialty, demonstrate experience in caring for patients with palliative care needs, and be able to meet the travel requirements.
The Specialist Palliative Care Team in CTMUHB is a multi-professional team providing clinical, advisory, and educational services across CTMUHB. The team is situated in locality bases, each housing a SPC inpatient unit and providing outreach services to both community and hospital patients.
