We have one full-time Band 6 Pharmacist position at Glangwili General Hospital and one full-time Band 6 Pharmacist position split between Glangwili General Hospital and Ceredigion Primary Care.
Successful candidates will be supported within a nurturing working environment by pharmacist mentors to develop their career portfolio and undertake the prescribing qualification. Prescribers currently practice within Rheumatology, Cancer services, Dermatology, Gastroenterology, Heart failure, and Frailty. You will be part of the planned digital transformation of medicines management systems and will be actively encouraged to engage with these processes to ensure its success.
Main duties of the job
One post will be based in Glangwili General Hospital and the other post will be split between Glangwili General Hospital and Primary care Ceredigion. Please specify in the first line of the supporting information on your application which you are interested in.

* The post holder will provide a safe and effective clinical pharmacy and medicines management service to clinical area(s) and advise medical staff and the wider MDT on pharmacy matters.
* Rotations may include (depending on base site), medicine, admissions, surgery, trauma & orthopaedics, frailty, critical care, paediatrics, and palliative care; there may be an opportunity for the post holder in primary care to rotate within secondary care services.
* The post holder must comply with current legislation and UHB policies. The recommendations contained within the Code of Ethics as issued by the General Pharmaceutical Council and the obligations in the Standards of Good Professional Practice will be upheld at all times.
* The post holder must undertake post-registration training identified by the service and individuals' needs.

Job responsibilities

To meet the criteria for the HEIW foundation place you must be qualifying in July 2025 or have registered with the GPhC after July 2024.
The post holder will be expected:
* To participate in the delivery and development of ward-based clinical pharmacy services.
* To participate in the provision of Medicines Information and advisory services to medical, nursing, and other staff.
* To provide clinical pharmacy support to the inpatient and outpatient dispensaries and to undertake the role of Responsible Pharmacist when required.
* To provide professional supervision to technical and support staff.
* To contribute to the training of Trainee Pharmacists and Pharmacy Technicians.
* To participate in clinical audit.
* To contribute to the practice and development of quality governance throughout the department.
* To undertake any training (managerial and professional) identified by staff appraisal.
* To participate in weekend, on-call, Bank Holiday, early, and late working in accordance with agreed rotas.
* To maintain confidentiality at all times relating to patients, staff, and the Trust.
* To actively participate in the Staff Appraisal Process.
* To be familiar and comply with relevant Operational, Personnel, Health and Safety policies and procedures, including Fire, COSHH, No Smoking, and Alcohol.
* Maintain a safe environment by adhering to local infection control policies.

Person Specification
Qualifications and Knowledge
* Masters degree in pharmacy (4 years or equivalent)
* Member of General Pharmaceutical Council (registered pharmacist or pre-registration pharmacy trainee during applications, must be registered pharmacist at time of start date)
* English language qualification - GCSE grade C or above or IELTS score of 6.5 in writing and 6.0 in all other components.
* Professional knowledge of medicines management systems and processes in an integrated NHS.
Experience
