Job overview
To work as a member of the Home Treatment and Rapid Response services, providing support to patients and their families within the home treatment function. This involves delivering evidence-based, profession-specific, and general mental health support, focusing on client-centered, recovery-oriented principles for individuals, families, and carers referred to the service. You will also work within the Rapid Response function, undertaking assessments 24/7 received from the Urgent Mental Health Helpline, including gatekeeping assessments. The role involves working in accordance with Trust and Crisis Resolution Home Treatment Team Policies and Procedures and the Care Programme Approach. The aim is to provide an alternative to inpatient admission by treating patients in their home environment during crises with significant distress. The service covers Dover/Deal, Shepway, and Ashford.
Main duties of the job
* Contribute to the assessment of clients referred to Home Treatment and Rapid Response.
* Provide support to clients and their carers concerning personal care, daily living, social activities, and maintaining social networks through various home treatment interventions.
* Carry out specified interventions independently as per crisis plans and update them as needed.
* Monitor clients' progress against crisis plans, communicate findings, and recommend changes to senior staff for review.
* Report any changes in client presentation and safety concerns to senior staff.
* Work flexibly to respond to crises, adjusting work and priorities as required by the Shift Co-Ordinator.
* Engage clients and carers with crisis plans, negotiate options, and promote crisis prevention, recovery, and resilience.
* Support carers to enable clients to remain at home, including assisting with medication administration as per crisis plans.
* Promote the work of Home Treatment and Rapid Response to clients, carers, mental health professionals, and other agencies.
