Job summary We are looking to recruit a Critical Care Outreach Practitioner on a permanent basis. We are looking for a compassionate, enthusiastic and driven individual who is passionate about making a difference to the care of acutely unwell and deteriorating patient's. Our CCOT is a proactive service that provides early recognition, assessment and management of the deteriorating patient trust wide. As a CCOP you are responsible for the delivery of patient focused programmes of care, which addresses the complex and changing health and well being needs of the deteriorating and critically ill patients throughout the trust. You will be part of a team at the forefront of developing a service to meet the future needs of the Trust. Main duties of the job Provide effective and efficient communication and working relationships with nursing colleagues and MDT members throughout the wards and departments and other hospitals when discussing clinical situations and when attending meetings. Provision of a rapid response and support system to allow the use of expert knowledge and in depth experience to identify deterioration of patients' physical condition from the NEWS system and as a result provide highly specialized intervention and advice with appropriate physical assessment, observation and treatment for acute patients on the ward. Act as an expert resource and point of referral for nursing in the specialty and advocate on behalf of critically ill patients, their families and the service as a whole. Discusses care of any critically sick patients and advises on treatment within the agreed protocols.Summarisesall patient assessment in medical notes and advises staff of recommendation made.
