Job Description

This is a full-time role, working 37 hours per week, on a Maternity Cover basis.
Day to Day Duties of the Role
* Proactively deliver the Specialist Support Model of inclusive practice and implement the SEND Code of Practice.
* Plan and review individual programmes of support ensuring all aspects of provision and support specified within a learner’s ILP and/or Education Health and Care Plan are in place and the progress is effectively tracked and monitored.
* Carry out high quality Initial Needs Assessments and diagnostic assessments along with other appropriate assessments to establish support needs and effectively track progress leading to successful outcomes.
* Arrange and deliver transition programmes to ensure seamless transitions between FE and HE.
Required Skills and Qualifications
* LDD Specialism e.g. foundation degree in LDD or other specialist qualification.
* Working knowledge of Additional Support Funding.
* Relevant and successful experience in learning support.
* Skills in the development and implementation of Individual learning programmes.
