Main duties of the job
As a Paramedic within our PCN, you will play a vital role in supporting our practices and patients, ensuring they receive the appropriate care promptly and effectively. You will work closely with our MDT and other healthcare professionals to provide holistic care for our community.
Key Responsibilities:
* Delivering high-quality care to our patient population compliant with the DES.
* Work collaboratively as part of a multi-disciplinary team with GPs, nurses, and other primary care professionals within the PCN.
* Contribute to the delivery of the PCN model, ensuring it reflects local priorities, addresses health inequalities, and supports population health management.
* Provide direct clinical care to patients, including assessment, treatment, and referral as appropriate.
* Collaborate with multidisciplinary teams to enhance service delivery and patient outcomes.
* Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.
* Maintain accurate health records appropriate to the consultation, ensuring completion of all necessary documentation associated with patient healthcare and registration with the organisation.
* Communicate effectively with patients and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their health needs, results, findings and treatment choices.

Job responsibilities
Key Responsibilities:
* Patient Care: Provide urgent, out-of-hospital care to patients as part of the PCN, assessing and managing a wide range of clinical conditions, delivering high-quality care to patients in their homes or in the GP practice.
* Collaborative Working: Work closely with GPs, nurses, care coordinators, clinical pharmacists, physiotherapists, and other healthcare professionals within the PCN to ensure the delivery of integrated, patient-centered care.
* Multidisciplinary Team Support: Work as part of a multidisciplinary team, to manage patients with acute concerns, complex health needs, frailty, and long-term conditions.
* Health Promotion and Prevention: Support proactive and preventative care, including identifying health risks, conducting health assessments, and offering advice on lifestyle changes to reduce future health issues.
* Health Inequalities & Population Health: Contribute to reducing health inequalities and ensuring that services meet local population health needs, particularly for vulnerable groups and those with chronic conditions.
* Clinical Support: Provide on-the-spot clinical assessments, treatment, and referrals for patients as required, ensuring they receive the right care at the right time.
* Documentation & Reporting: Maintain accurate, detailed records of patient care and interventions in line with NHS and PCN standards. Ensure all documentation is up-to-date and compliant with governance and legal requirements.
* Training and Development: Engage in ongoing professional development and contribute to the continuous improvement of the service, including attending training sessions and team meetings.
Person Specification
Experience
* Recognise and work within own competence and professional code of conduct as regulated by the HCPC and College of Paramedics. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Be aware of data protection (GDPR) and confidentiality issues particularly within a GP surgery.
* Experience working in Primary Care.
* Experience working with EMIS.
Qualifications
* Paramedic Qualification.
* HCPC Registration.
* Prescriber.
