An opportunity has arisen for a highly motivated nurse to join us. This is an excellent opportunity for you to use your clinical and leadership skills to support the team.
The service provides acute (short-term) care in patients' own homes and Residential Care Home settings. The team promotes a self-management approach to patients with their health conditions.
A key priority is for rapid nursing staff to educate patients on their health needs and to provide a self-management approach to patients with their condition.
The service provides cover 7 days per week, 08:00-18:30 and incorporates a rapid response service supporting patients with urgent nursing care needs.
You will have the opportunity to develop your skills of specialist nursing assessment and treatment in a community setting, alongside developing additional generic skills to provide a comprehensive assessment to a variety of complex patients.
You will be well supported in the role by a skilled multi-disciplinary team.
Main duties of the job
Some of your responsibilities would include:
1. Urgent (same day) community nursing referrals
2. Coordinating the nursing response
3. Responding to patients in immediate need
4. Clinical triage of referrals
5. Managing the caseload
6. Leadership of staff
7. Developing service and supporting your colleagues

* Relevant nursing degree or equivalent relevant experience
