Northumberland Park Medical Group are looking to recruit a Full time Healthcare Assistant to join the practice team.
We are looking for a highly motivated individual, with experience of working in primary care to work alongside our existing Nursing team.
Chronic Disease Management with a knowledge of QOF and Primary Care experience is desirable.
Working hours are 37.5 per week over 4 or 5 days. Working days will include Monday's and Friday's.
Main duties of the job
The successful candidate will provide a range of Healthcare Assistant duties including (but not limited to) Phlebotomy, B12 Injections, Chronic Disease Management initial information gathering, New patient health checks, NHS Healthchecks, BP Checks, Diabetic Foot Screening.

Job summary:
Working under the direct supervision of the Partners and strictly in accordance with specific practice guidelines and protocols, the HealthCare Assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care.
Duties and responsibilities: Clinical
* New patient health checks.
* Urinalysis.
* Weight check and BMI recording.
* Administration of Flu and Pneumococcal vaccinations and Vitamin B12 injections in line with patient specific direction.
* Supporting Practice Nurses when required.
* Other health checks (i.e. well person checks).
* Long Term Condition Reviews.
* Participation in general health promotion and lifestyle advice.
* Act as chaperone as required.
* Processing and management of laboratory samples requested by GPs or nurses.
* Any other duties and responsibilities appropriate to this post as delegated by the Lead Practice Nurse, Practice manager or GPs.
Administration and Professional Responsibilities:
* Vaccine and cold chain storage, monitoring and recording.
* Organisation of the restocking and maintenance of clinical areas and consulting rooms.
* Preparing and maintaining environments and equipment before, during and after patient care interventions.
* Conduct monthly and annual stock take and submission of records to assist the Practice Manager.
* Calibration and quality control of clinical electrical equipment.
* Attend and participate in Practice Clinical and other meetings as required.
