This investment has enabled us to expand our Hospital Liaison Mental Health Team. Liaison Mental Health Service Model within Doncaster Royal Infirmary is part of NHS England's 'Achieving Better Access to 24/7 Urgent and Emergency Mental Health Care' Guidance. We are looking for a highly skilled and motivated Mental Health Nurse to join our Older Persons Hospital Liaison Team. To be successful in meeting the challenges involved in the role, you must be able to work autonomously and have strong clinical and risk management skills. Part of the day to day role is to support the general hospital wards with Older Persons mental health, assessing, continuing care and planning of a safe discharge. Main duties of the job Provide a response to Mental Health Crisis in Emergency Departments and Inpatient Wards within one hour and to all urgent Ward referrals within 24 hours. Complete a full biopsychosocial assessment and formulation and contribute to treatment and collaborative care plans. Offer brief evidence-based interventions as inpatient or short-term outpatient follow-up.
