An exciting opportunity to join our Home First Rehab & Reablement team. Our service covers the whole of Salford facilitating clients discharge back into their own home. Our vision for Home First is 'People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary.'
We are seeking caring and compassionate people who enjoy interacting and assisting people in their own homes with their daily living activities. You will be an excellent team player and possess excellent communication skills, able to work on your own initiative. Candidates must be warm and friendly, with a strong work ethic, a positive attitude, and a genuine passion for providing high-quality care and support for the people of Salford.
Due to the role working in the community, a Full Driving Licence and the use of your own vehicle is essential.
The Home First Reablement Service operates 7 days a week from 8am to 10pm, and various shift working is available. The successful applicants will work on a rota that covers shifts, including weekends and Bank Holidays.
Main duties of the job
To provide support and person-focused re-ablement as required to help ensure the efficient and effective running of the service. Duties may include:
* Assist the individual with personal care tasks, such as washing, dressing, meal & fluid prep, toileting, and administering/reminding medications.
* Assist in the safe lifting, transferring, repositioning, and movement of the individual.
* Support individuals to live independently in their own homes and help achieve their own outcomes.
* Observe, monitor, and record clients' physical and emotional well-being, and promptly report any changes to senior staff.
* Record all visits and care on the Electronic Patients Record (EPR), Liquid Logic, and in the Patients' personal folder.
* Encourage clients to participate in social and recreational activities.
* Attend relevant training for the post.
* Attend Daily Huddles.

Person Specification
Qualifications
* Level 1 Literacy & Numeracy
* NVQ Level 2 or Care Certificate (or equivalent experience - in either Therapy or Health Care)
Knowledge, Skills and Experience
* Full Driving Licence is required and the use of own vehicle.
* Good communication skills / the ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post.
* Understanding of routine work procedures gained through a short induction period and on-the-job training.
* Experience of working with members of the public or a desire to work in the Health Service.
* Basic IT skills.
* Experience of working within a health or social care environment.
* Experience of working within a team.
* Good organisational skills.
