Job summary
Southern Brooks is seeking a Team Leader to guide a small team of inspiring and dedicated Social Prescribing Link Workers, based in Kingswood, South Gloucestershire. This is a full-time 37 hours per week role, however, applications for 30 hours per week will be considered.
Social Prescribing Link Workers work closely with primary care teams to enable individuals to find the support they need in their community to manage their health and well-being. They see people in their local GP practice and community venues and may visit people in their homes, and enable to have more control over their lives, develop skills and give their time to others through involvement in community groups.

Main duties of the job
As a Team Leader you will lead a small team of Social Prescribing Link Workers within a Primary Care Network (PCN). You will support the Social Prescribing Manager in managing relationships with partners as well as holding a caseload.
In your role as Team Leader, you can expect to:
1. Provide line management and supervision to a small team of Social Prescribing Link Workers within Network 4 PCN.
2. Assist Link Workers in building relationships with GP surgeries to establish a social prescribing service within the surgery.
3. Support Link Workers to build expertise, resilience, and confidence in themselves.
4. Review ways of working to provide continuous improvement for the service across the PCN

6. Link Workers give people time to talk about what matters to them and guide them to unpick complex issues affecting their health and wellbeing. They support people to find services and communities that will support or fulfil them. Link Workers will see people in their local GP practice and community venues and may also do home visits
7. As a Team Leader you will lead a small team of Social Prescribing Link Workers within a Primary Care Network. You will support the Social Prescribing Manager in managing relationships with partners as well as holding a caseload

8. Provide line management and supervision to a small team ofSocial Prescribing Link Workers within Network 4 PCN
9. Support the induction of Network 4 Link Workers
10. Assist Link Workers in building relationshipswith GP surgeries to establish a social prescribing service within the surgery
11. Support Link Workers to build expertise, resilience, andconfidence in themselves
12. Coach Link Workers to manage their caseloads
13. Review the performance of individual Link Workers and theteam against key performance indicators
14. Ensure Link Workers risk assess cases and follow theappropriate safeguarding policies and procedures
15. Review ways of working to provide continuous improvement forthe service across the PCN
16. Collect and collate data to enable effectivereview of the service
17. Identify when a person's needs are beyond the scope of thelink worker role and refer to the appropriate service
Working as part of a Multi-Disciplinary Team in GPsurgeries, Link Workers will
18. receive referrals from GPs
19. support individuals for 4-6 sessions, over 2-3 months
20. manage a caseload of around 10-12 cases
Provide personalised support
21. Using motivational interviewing and other techniques toestablish the needs of the individual and provide personalised advice
22. Support individuals to address barriers to accessingservices or maintaining independence
23. Empower individuals to maximise their potential, producingaction plans and facilitating follow-through
24. Accompany individuals to community groups, activities, andstatutory services. Following up to ensure they are happy and supported, ableto engage, included and receiving good support
25. Follow safeguarding protocols when needed
Communication and Networking
26. Develop and maintain good relations with GP practice teams,Southern Brooks teams, and external practitioners. Including participating inteam meetings, including multi-disciplinary, and working alongside the widerSouthern Brooks team
27. Positively promote the service to practice teams, externalagencies, and relevant statutory bodies
28. Contributing to a joined up approach to client action plans
29. Seek support from Manager and Peers when there are gaps inknowledge or practice limitations
30. Actively seek connections and build relationships betweenpeople and groups outside of Southern Brooks
31. Seek opportunities to link individuals to activities,supporting them to develop relationships and build their own social networks
32. Identify gaps in local provision based on the needs ofindividuals, including opportunities to establish new activities, groups, orintroduce new funded projects. Inform the manager of these gaps andopportunities
33. Assist with community resource mapping exercises and sharewith wider teams in Southern Brooks
34. Maintain accurate case files and records in line with alloperational procedures and produce written reports if required
35. Produce case studies to evidence the impact of the serviceon an individual's wellbeing
Person Specification
Qualifications
Essential
36. Knowledge of and experience in client-working techniques such as motivational interviewing and active listening. This includes strength-based, trauma-aware and person-centred approaches.
37. Educated to GCSE Grade C or above (or equivalent) in English and Maths.
Desirable
38. NVQ level 2 in social care or equivalent experience.

39. Leadership, coaching and management of team members and/or teams.
40. At least 2 years experience of working with people with health and social needs, or community-based work with socially excluded groups
41. Experience of managing own caseload
42. Good understanding of the wider determinants of health and wellbeing
43. Knowledge of brief interventions, behaviour change, motivational change.
44. An understanding of risk assessment, risk management and safeguarding vulnerable people.
Desirable
45. Experience of working with a wide range of people, including people from different cultures, older people, and those with health conditions / disabilities and neurodivergence.
46. Experience of using databases to record client actions and interventions.
