Job summary The Hospital at Home service is expanding and has vacancies for the newly created Health and Care Assistant roles. The successful candidates will work alongside the Discharge Home to Assess team to predominately provide short term Care to prevent patients requiring hospital admission. The roles are based within the East and North corridors and staff will be expected to travel within the corridors, Previous care experience is essential. The service runs 7 days a week, 8am to 8pm.
