We are seeking a suitably qualified Band 6 nurse to join the Assertive Outreach Team (AOT) here in Sheffield. This is an exciting opportunity for the right candidate to positively engage with and change the lives of service users who have a severe and enduring mental illness and who have disengaged with mental health services.
The team is funded to provide a 5-day-a-week service, with the potential to develop into a 7-day service, depending on additional funding. The successful candidate will need to be available to work flexibly alongside the development of this team.
The AOT offers a city-wide service aimed at working with physical health, social, and economic services to reduce exclusion and promote the psychological needs of service users who often face barriers to accessing services.
You will need to have enthusiasm for, and experience of, working with service users with complex needs who experience difficulty in engaging with mainstream services.
You will be responsible for setting supervision and support systems for junior AOT staff in their delivery of interventions and for delivering excellent evidence-based care to patients within the service.
Main duties of the job
* The post holder will work as a Band 6 Lead Nurse within the team, responsible for the coordination of care and support for an identified caseload of service users.
* Maintain NMC registration/re-validation and adhere to professional guidelines and code of conduct.
* Work within the framework of relevant local and national policy and legislation.
* Provide supervision to junior members of the staff team.
* Responsible for the planning, implementation, and evaluation of nursing care within agreed parameters, with a working knowledge of local and national policies and legislation.
* As an active member of the team, undertake the assessment and screening of patients referred to the AOT service.
* Ensure effective communication pathways exist with both internal and external services.
* Promote equality, diversity, and rights in accordance with good practice and legislation.
* Identify and contribute to the training needs of themselves and those they supervise, necessary for providing quality evidence-based care.
* Support colleagues in the effective assessment and delivery of interventions to individuals within their assigned caseload.

Job responsibilities
Clinical:
* Undertake comprehensive assessments of individuals' health and social care needs in various settings and agree on the most appropriate course of action.
* Assess, evaluate, and manage clinical risk, escalating concerns appropriately when required.
* Formulate a plan of care based on identified needs.
* Deliver evidence-based interventions as appropriate.
* Review the effectiveness of care plans at regular intervals.
* Work collaboratively with service users, carers, and partner agencies.
* Maintain computer-held care records accurately and timely.
* Undertake all work in a culturally sensitive manner.
* Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
* Develop effective working relationships with Primary Care colleagues.
* Undertake Team Management role in the absence of the Team Manager.
Professional:
* Participate in the Trust appraisal process and take a lead in identifying and engaging regarding personal development, professional mandatory & supervisory training needs.

Supervision:
* Regularly review own caseload with a supervisor to promote efficacy and contribute to the overall management of the Service.
* Deliver supervision and facilitate PDR with more junior members of staff.
* Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve competence and effective contribution.
Person Specification
Technical
* Knowledge and understanding of appropriate therapeutic approaches and engagement strategies.
Experience
* Experience of multi-disciplinary team working.
* Extensive experience in mental health, personality disorder, and learning disabilities in both inpatient and community settings.
* Experience of working in a psychologically informed way through case formulation.
Knowledge
* Ability to work autonomously and make decisions independently based on a thorough understanding of care plans.
* Supervision training.
