Daytime Healthcare is working with an established Home Care agency, who are expanding and looking for an experienced Care Coordinator to join their friendly team!

Duties include:
1. Responsible for carrying out initial assessments, doing reviews, and spot checks.
2. Creating service user care plans and planning staff rotas using the bespoke system.
3. Supporting the new and existing care staff team and coordinating the key carers in your designated areas.
4. Ensuring that all operations are Company and CQC Compliant.
Requirements:
1. You must have a full valid Driving Licence with access to a vehicle.
2. Good organisational and communication skills to ensure an excellent service.
3. Experience in doing rotas and care plans.
