Provide a high standard Speech and Language Therapy service, assessing and advising on patients care & treatment, as an autonomous practitioner working with patients with dysphagia and/or communication difficulties as a result of head and neck cancer. Manage a caseload of patients with varying diagnoses and complex needs relative to your experience including using evidence-based client-centred principles. Support the development of junior staff members through case discussions, supervision and training depending on experience. Maintain clinical records in line with local / Trust documentation guidelines. Work as an integral part of the multidisciplinary team, contributing to team discussions and influencing decisions about patient care programmes, attending Multi-Disciplinary Meetings, board rounds, and family meetings where necessary. Contribute to maintenance and development of the SLT service. Participate in information management to ensure high quality data which is recorded, evaluated & reported in a timely manner. To work within National and Trust policies / guidelines; and HCPC and RCSLT standards and codes of conduct. To have a solid understanding of national and local standards and be able to monitor own practice against these. To participate in bank holiday working and work flexible hours to meet the needs of the patients if required. Key Responsibilities Undertake comprehensive assessment of patients with dysphagia and/or communication difficulties including those with complex presentations. Investigate & analyze skills & formulate individualized management & treatment plans and specialized programmes of care. Interpret & analyze clinical & non-clinical facts to form accurate diagnosis & prognosis on a wide range of highly complex conditions. Evaluate patient progress, reassess, and alter treatment plans as required. Accept clinical responsibility for the dysphagia caseload and organize this effectively and efficiently. Be professionally & legally responsible & accountable for all aspects of your own work. Undertake department audit and service development work, translating findings into practice. Practice Abide by the Royal College of Speech and Language Therapists & the Health Professions Council standards of proficiency. Be responsible for own personal development to establish and maintain competence and fitness to practice. Establish effective communication networks for dysphagia with patients, carers, MDT, other workers and agencies across health and social care. Identify and understand risks and benefits of own practice. Seek support from senior colleagues and peers when necessary. Aware of local risk escalation and reporting process. Comply with organisational, professional & Trust policies & procedures. Communicate effectively, plan, organise & work collaboratively with medical, nursing & therapy colleagues. Provide person-centred SLT care considering the wishes of the service user. Document patient activity data in accordance with Trust and professional standards.
