Main duties of the job
* Providing pharmaceutical care to patients from admission to discharge on the wards
* Actively participating as part of the multi-disciplinary team at ward level
* Providing prescribing advice and support to medical staff
* Educating patients about their medicines
* Participation in quality improvement projects across the department and clinical areas
* Participating in late night and weekend working and on-call rotas

Job responsibilities
* To clinically review prescriptions in both the ward and dispensary environments.
* To provide specialist pharmaceutical advice
* To accurately dispense and check prescriptions in the dispensary
* To conduct medication reviews at admission and discharge and implement changes to medicines as necessary and to accurately communicate these changes to primary care colleagues
Trainee pharmacists who plan to qualify in summer 2023 will be considered for this post.
Person Specification
Qualifications
* GPhC registered/GPhC provisional registration
* Master Degree in Pharmacy (4yrs) or equivalent
* MRPharmS
Experience
* Experience in working in multidisciplinary teams
* Hospital pharmacy experience
* Community pharmacy experience

Salary: £37,338 to £44,962 a year (pro rata)
