Role Summary:
As a Practice Manager, you will report to a Team Manager and take on responsibility for managing and supervising a team of qualified and support staff. You will also support colleagues in delivering exceptional care and ensuring legal and regulatory compliance.
Your responsibilities will include:
1. Managing a complex caseload and triages.
2. Leading as a Duty Lead within the team.
3. Developing person-led support plans for adults in Mental Health and Wellness.
4. Working with vulnerable adults and older people, ensuring their safety, independence, and well-being.
5. Coordinating with external partners and agencies to deliver integrated care services.
6. Representing the Council at multi-agency meetings.
7. Managing a team of staff, offering guidance and supervision to ensure high performance.
8. Undertake the full legal range of social care assessments and develop person-led support plans.
9. Maintain good working relationships with professional colleagues, external agencies, and service users.
10. Demonstrate a high level of communication, negotiation, and assertiveness skills.
11. Handle challenging situations, ensuring the safety and well-being of adults and older people.
12. Provide direct line management and supervision for staff members.
Skills and Experience Required:
1. Significant post-qualifying social work experience in a children and families or mental health setting.
2. Experience of complex case management and risk assessment.
3. Ability to make decisions based on thorough analysis and evidence.
4. Demonstrable knowledge of working with multi-cultural communities and delivering culturally sensitive services.
5. ICT proficiency, including Microsoft applications.
6. Proven experience managing complex and challenging situations.
Qualification:
1. Social Work Degree (BA or MA in Social Work).
2. HCPC Registration as a qualified social worker.
3. AMHP (Approved Mental Health Professional) Qualification.
4. Post-qualifying experience in a relevant social care or mental health setting.
5. Enhanced DBS Check (Disclosure and Barring Service).
