Job Location: Grimsby (North East Lincolnshire) with travel around the area
Salary: £23,745 per annum pro rata
Hours: 35 per week
Contract Type: Fixed Term Contract Until 31st September 2025
A Full UK manual driving licence and access to vehicle required for this role. (Business mileage paid)

What will a day in the life of a Service Coordinator entail?
* You will manage a small team of staff and volunteers ensuring the people who need us receive the best possible service.
* You will meet with our service users and assess their needs, guiding or signposting them to connect with groups or to manage a health condition. You'll enable them to lead independent lives and improve their wellbeing.
* You will liaise with partnership agencies to promote the service and build meaningful links.
* You will produce and maintain monthly reports.
What do you need to be a successful Service Coordinator?
* Do you have strong leadership skills and a knack to get the best out of people?
* Are you a good communicator?
* Do you have knowledge of Health and Social Care services?
* Do you have a manual UK driving licence, held for a minimum of two years, and access to a vehicle? (mileage will be paid).
