Job Overview:
We are looking for an experienced and motivated Band 6 Physiotherapist to join the community team at University Hospitals Coventry and Warwickshire NHS Trust. In this role, you will deliver high-quality physiotherapy services to patients in the community, managing a caseload of individuals with a variety of musculoskeletal and rehabilitation needs. You will be responsible for conducting assessments, developing treatment plans, and supporting patients with their rehabilitation to improve mobility and independence.
Key Responsibilities:
1. Manage a community caseload, providing physiotherapy assessments and treatment plans for patients with musculoskeletal and rehabilitation needs.
2. Work collaboratively with multidisciplinary teams to ensure the best outcomes for patients.
3. Provide advice and education to patients and their families about self-management and rehabilitation strategies.
4. Develop and implement evidence-based interventions for a diverse range of conditions.
5. Maintain accurate and up-to-date patient records and documentation.
6. Contribute to the development of services and quality improvement initiatives within the team.
Key Requirements:
1. HCPC registration.
2. Minimum 6 months of post-graduate experience in community physiotherapy or relevant clinical settings.
3. Strong experience in musculoskeletal rehabilitation and complex case management.
4. Excellent communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Driving license and access to a car for community visits.
