About the Role:
As a Social Worker in our Mental Health Team, you will play a vital role in supporting individuals with mental health needs. Your responsibilities will include:
* Managing a complex caseload of individuals with a range of mental health conditions.
* Conducting comprehensive assessments, including mental health assessments, and developing personalised support plans.
* Working collaboratively with service users, their families, and carers, promoting self-advocacy and empowerment.
* Liaising and collaborating with a range of partner agencies, including mental health services, community health teams, housing providers, and voluntary organisations.
* Providing crisis intervention and support as needed.
* Safeguarding vulnerable adults and adhering to all relevant legislation and policies.
* Maintaining accurate and up-to-date records.
About You:
* Qualified Social Worker registered with Social Work England.
* Demonstrable experience of working with individuals with mental health needs.
* Sound knowledge of relevant legislation and guidance, including the Mental Health Act 1983 and the Care Act 2014.
* Excellent assessment, analytical, and communication skills.
* Ability to work independently and as part of a multi-disciplinary team.
* A commitment to promoting recovery and wellbeing.
* Strong interpersonal skills and the ability to build rapport with service users.
