Colchester Medical Practice has an opportunity for a health and wellbeing coach join our primary care network (PCN), to support our four GP practices across the Colchester area, to assist in re-shaping primary care across our communities and empower our patients. This role is offered on a full-time basis. Health and wellbeing coaches partner with individuals to guide them through choices according to clinical information, giving the individual control over their own health. Main duties of the role: use health coaching skills to support people to become active in their care and reach their health and wellbeing goals. help individuals to improve their skills, knowledge and confidence so they can make informed decisions about their care. provide access to support infrastructure such as education, support networks and further social prescribing as necessary. help patients self-identify issues and encourage active prevention of negative health impacts. motivate and support patients as they implement a personalised health plan. focus on lifestyle behaviours to reduce risks, improve self-management of chronic conditions and enhance over-all quality of life. Other duties will include: care-planning and the delivery of systematic self-management support, based on the knowledge of each individual’s activation levels; care-coordination across the care continuum (including identification and support of carers); provide personalised support through face-to-face, telephone, and digital coaching tailored to each individual’s specific needs, location, and preferences. supporting effective team-working in primary care, through taking on appropriate practice-based tasks, attending regular team meetings, if required, and working with the complex-care teams; delivering group and one-to-one sessions on healthy eating, physical activity and health and wellbeing, to tie-in with personalised care plans; engaging patients, and using health-coaching techniques, to support them to take an active role in their health and wellbeing; helping to raise the local population’s awareness of the support groups and the opportunities available to assist them in achieving their health and wellbeing goals; working in partnership with clinical and non-clinical colleagues, management support and the wider PCN, to ensure the role delivers the best possible outcomes for our patients. About the Candidate The ideal candidate will: Be an experienced health and wellbeing professional, confident to deliver support and advice to a range of patients with diverse health needs. Have experience in supporting patients with long-term conditions, to improve and manage their own health and wellbeing effectively. Be experienced in delivering personalised care plans. Be confident to write concise, accurate and timely bespoke care assessments. Be experienced in teaching patients and their carers, to understand and manage their own conditions in order to maintain an independent lifestyle. Have excellent interpersonal and communication skills. Be confident to be involved in multidisciplinary meetings. Have strong local knowledge, in order to promote local support networks and services in our community.
