* Your mission: To provide expert interventions and rehabilitation as part of a dynamic, tight-knit team that reacts quickly when people are in crisis.
* Your aim: To help patients stay in their own home, empowering recovery and preventing unnecessary hospitalisation. You'll also support early discharge from hospital and patients at the end of life.
* Your role: Add your expertise to our multi-disciplinary community team as a Physiotherapist, Occupational Therapist, Nurse, Paramedic, Community Healthcare Assistant Practitioner or Healthcare Assistant.

Main duties of the job
#MissionRRIC
* Providing advanced skilled, professional assessment of health needs and relevant therapy case management, especially for those with complex needs and high risk of unplanned admission to hospital.
* Educating and supporting members of the multi-disciplinary team to intensively case manage these patients.
* Working with a range of professionals across the system to ensure care is appropriate and delivered in a timely manner with the aim of preventing hospital admission, supporting early discharge and reducing GP contact.
* Demonstrating your professional skills and competence in leadership, care planning/co-ordination, and care and treatment delivery.
* Engaging with RRIC Clinical Leads to offer and find ways for continuous service development and deputise for the Clinical Lead as needed.

Person Specification
EDUCATION, QUALIFICATIONS & TRAINING
* Registered Allied Healthcare Professional with HCPC - Physiotherapist / Occupational Therapist / Paramedic or Registered Nurse with RCN
* Degree/ Diploma in Physiotherapy, Occupational Therapy, Nursing, or Paramedic Science
* Recognised teaching or mentorship qualification equivalent to level 3
* Recognised management/ leadership qualification
* Post graduate modules
EXPERIENCE
* Relevant specialist/clinical experience and competence
* Experience of working as advanced clinical practitioner/case manager
* Management of complex Long-Term Conditions
* Knowledge of managing cognitive impairment and mental well-being
* Supporting self-care, self-management and enabling independence
* Developing highly specialised programme of care, providing advice concerning treatment of patients
* Professional practice and leadership
* Managing care at end of life
* Experience of interagency and partnership working
SKILLS, ABILITIES & KNOWLEDGE
* Highly specialist knowledge in managing patients in the community with complex medical needs, long-term/chronic disease management and palliative care
* Highly developed professional knowledge base supported by educational development and different work environment.

* Highly developed general management practice, including organisation performance management finance, human resources and equality and diversity
